Welcome to Cohen and Montini Orthodontics located in both Ocala and Gainesville, Florida. Our orthodontists, Dr. David A. Cohen and Dr. Reid W. Montini, are committed to excellence by using only the latest technology in both clinical and business aspects. Both of our doctors strive in creating a comforting atmosphere for adults and children, and our staff members are trained and certified to offer our patients the best in dental care. Our offices are patient-friendly and we offer a wide range of orthodontic services, including the newest technology in orthodontics; Invisalign. We also continue to offer silver and gold braces, and for a more natural look, the Clarity ceramic brackets. Our initial consults are complimentary with a full dental screening and information about braces. New Patients:

A couple weeks before the consultation appointment, all new patients will receive a "new patient info package" from our office. Please read the information provided about braces and bring the filled paperwork to your consultation appointment. If you have received the package but misplaced the paperwork, feel free to download your paperwork by clicking on the appropriate file below. These files are our office's medical history forms for both adults and children. The file will download in the same browser window and before you print the paperwork you made need to resize the document to about 75%, to get most of it to fit on the paper. Once again, please bring this form filled out to your orthodontic consultation.
